Complete the case study found in “Health informatics an  Interprofessional Approach” by reading the case and answering the  questions. In addition, design a form for capturing data when the  Emergency Department registration system is not available. Your case  study must include:

• A minimum of 20 data elements
• A data dictionary outlining: 
  • Data field name
  • Field format (alpha, numeric or alpha numeric)
  • Number of characters in the filed
  • Source of information
  • Example of the content

Your response to the case study questions should be a minimum  of one full page in length,  excluding the title and reference pages,  and be formatted according to APA guidelines as outlined in the Ashford  Writing Center.
